If you purchased elsewhere, to receive your free digital forms by direct download or CD email your Purchase Receipt to publisher, Business Source Publishing Co. at bzdealman@gmail.com stating your choice of either a direct download or CD. Your digital forms will be emailed to the email address you provide or mailed to the address you include in your email if you want the physical CD instead. If the Forms are emailed to you you will not receive the physical Forms CD.
